-Nominations
should demonstrate the impact that the individual or organisation has
had on
the equality and human rights of disabled people, i.e. anyone living
with
ill-health, injury or disability.

nomination should be no more 1000 words in
total. You may wish to concentrate on a
specific area, or a few different projects, policies or initiatives.

-It
is important to include strong evidence of targets achieved, results or
impacts
of work, and details of how the delivery was accomplished.
This may include the time-scale, the
funders, the barriers that were overcome, statistical evidence or
perhaps a
testimony from those involved or the beneficiaries.

-Please
be aware that we will use the written submission without supporting
information
when short listing and judging the winner.
So, please include all the important facts, figures and details in your
statement.
